Information for Student-Athletes & Parents
As per ISU and NCAA Rules, all student-athletes must have satisfactorily completed an athletic health pre-participation physical examination within six months prior to participation with ISU athletic teams. There are also a group of forms that will need to be completed to accompany the physical examination prior to being cleared to participate. Participation with any ISU athletic teams is prohibited until completion of all required medical documentation.
