MEDICAL EXPENSES CONSIDERED
NON-PERMISSIBLE BY IOWA STATE ATHLETICS DEPARTMENT

- Any medical services such as consults, office visits, labs, xrays, MRIs, etc. received without a written referral from an athletic training staff member.
- Any prescriptions (with the exception of authorized refills) filled without a written referral from an athletic training staff member.
- Surgical expenses to treat a student-athlete's illness or injury that was not a result of practice for or participation in intercollegiate athletics at the institution.
- Medical or hospital expenses incurred as a result of an injury while going to or from class or while participating in classroom requirements (i.e., physical education).
- Emergency room visits that are not related to practice or competition.
- Any condition that is related to an accident (automobile or other) outside sanctioned athletic participation (i.e., pickup games even though the activity may be related to athletes sport).
- Surgery or treatment for any type of congenital disorder, illness, or any preexisting conditions (i.e., high school injury).
- Immunizations required by the institution (Iowa State University) for enrollment into the university.
- Dental work that you did not see an athletic trainer first and/or did not receive a referral. The Athletic Department is not responsible for dental problems resulting from poor personal hygiene or neglect. If you have a dental problem that is not the result of an athletic injury, you may be eligible for a fund supplied by the NCAA.
